It is said that the staff has the power over the ten plagues, but Naomi said that angels were responsable for killing the egyptian first borns, also saying Castiel was among them.

The 'Ailment Manipulation' refers to the staff's power to cause any of the ten Egyptian plagues on beings. 'Ailment' refers to an illness. Dtol (talk) 13:55, September 20, 2017 (UTC)
