The episode title refers to Jack being locked in the Ma'lak Box during the episode.
This is the sixth episode of the season to feature all main characters with Stranger in a Strange Land, Gods and Monsters, Unhuman Nature, Game Night and Absence being the first five.
This is the first time Alternate Bobby Singer has been in the bunker since Gods and Monsters.
It is revealed in this episode that Duma is currently the Ruler of Heaven and has been since The Shadow attacked Heaven in Byzantium.
Duma says that Jack could create angels by taking out a human soul out of its body and "forge" them to become one.
Duma confirms that John and Mary Winchester are in fact sharing a Heaven.
Lucifer’s original sin was taking humans and twisting their souls, turning them into demons. In this episode, Jack took humans and reshaped their souls to turn them into angels.
Harrison Tate was turned into a pillar of salt by Jack. This is similar to what Balthazar did in The Third Man.
