Death is the second of The Four Horsemen of the Apocalypse. Of all the riders, he is the only one to whom the text itself gives a name. Unlike the other three, he carries no weapon or other object (but some versions say that Death carries the scythe, symbolizing that he is the Grim Reaper); instead he is followed by Hades.

Death didn't make a physical appearance in "Abandon All Hope...", but his presence was released from its prison in this episode. Lucifer released Death with his ritual.

Normally Death is kept locked in a magical box/coffin 600 feet under the Earth as his power is apparently too dangerous to allow him to roam freely. Heaven only released him when God had something major planned such has the Great Flood, or as Bobby Singer put it "The last time they hauled him up, Noah was building a boat!"

Death is considered the father of all reapers and the creatures who are normally neutral, since death is impartial, all gathered in the town where Lucifer was going to release Death in order to serve him.

In "Dead Men Don't Wear Plaid", Death arrives in Bobby's home town and proceeds to revive the dead in the local cemetery. While still unseen, his presence is heralded by fierce lighting storms and unlike his siblings he still seems to ride a horse, as the sound of one panting can be heard in the graveyard before the dead rose. Unlike the reapers, Death's power to revive the dead is much stronger. He can revive dead people who have been cremated, such has Bobby's wife.

Those who are revived by Death at first appear normal but this is a ruse on Death's part so people will lower their guard around them. After five days the revived devolve in to stereotypical zombie behavior who must then be killed with a shot to the head. It was learned at the end of the episode that the entire incident was organized to attack Bobby because he was one of the few remaining elements keeping Sam from saying "yes" to Lucifer.

Though unseen by the audience, Bobby's wife met him when she came out of the grave. He was waiting for her so she could pass a message on that Bobby was the target of the attack. He was described by Bobby's wife has being very thin like a skeleton.
