When Gary, Trevor and Nora were messing around with dark forces they learned of the bounty that Hell had put on Dean's head, and when Gary spotted Sam and Dean, he came up with the idea of using a body swapping spell as means to get close to Dean and kill him for the bounty. Procedure[]
At the beginning of the episode, Gary and Sam have been swapped. It's unknown how and when Gary used this spell.
At the end of this episode, some details of this spell had been shown. This spell needed some unknown ingredients (details of the spell were not revealed in the episode). However, after the ingredients were mixed together in a bowl, a spell was recited.

After the spell was recited, Gary dropped some powder into the bowl between him and Sam, causing light to flare from it. When the light died down, we saw that Gary and Sam had returned to their normal bodies.
